1.  How do I set up/change/cancel an appointment?

2.  Do I have to pay for my visit at the time of my appointment?

We will work with your insurance company to file a claim for covered services.  You may be responsible for paying a portion of your deductible or co-pay, depending on your policy.

3.  With which insurance carriers do you participate?

John Deere, Tricare/Champus, PHCS, PHP TennCare, One Health, Trigon/Anthem, Aetna, PPO/USA, BlueCross/BlueShield, Cigna, Medicare, Medicaid and others.  Please call Charity Odom or Amy Tucker in our business office for specific questions.

4.  How can I get a prescription refilled or get test results?
